The challenge is a Google Form feedback survey. Fill it out randomly; none of the answers matter. On the last page before submission, there is a checkbox question titled 2->4->3->1 with four options:

• thx_4_play1ng
• _sekaictf

The title gives the order: pick option 2, then 4, then 3, then 1. Concatenating them gives the flag.
